Well? I personally take:

red theraband - for shoulder warm ups
wrist wraps - for benching
hockey ball - for soft tissue work on my shoulder
chalk - for my grip. I would say this the the most important thing
fat gripz - I use them for volume barbell stuff, feels much better on your joints
elbow sleeve - left elbow can be a bit funny, I think this is more of a psychological crutch than anything else

I have:
a roll of medical tape since my calluses have been known to rip.
1 lifting strap. (I usually lose the pair, this time I only lost 1.
3 sets of resistance band, light, medium and heavy. I've lost one of the light bands.
A rubber mat to put under my shoulders. I can't say it helps much but it doesn't take any space.
I used to carry chalk but most places won't let me use it anyway.

I also carry complete toiletries since my gym bag becomes my carry on if I have to travel. I hate packing, this way, I'm always ready to go.

In my gym bag:
Light EliteFTS band
Sheik 6" belt
all black vans slip ons(great for squats/dl's)
spare glasses(old school ray bans do not stay on my face for some reason)
ipod
lifting straps(which I don't use since I discovered mixed grip, but nice to have in case I want to do heavy shrugs)
baby wipes
shorts
long athletic pants
knee sleeves
gym towel

I think I'm going to start bringing chalk since they don't seem to mind.

I have my shoes with me. Nothing much more. On the deadlift day I carry and use chalk, but nothing else is yet needed. Maybe taking rubber band sometimes. Oh, and a water bottle and a log journal of course.
On the good side, I dumped the gym gloves as nearly useless piece of equipment.

My gym bag lives at the gym when I'm home in PNG, since we don't have lockers, but travels from gym to gym with me when I'm in the US/Canada.

Athletic tape
Bottle of rubbing alcohol
Small bottle of tincture of benzoin
Gauze
Chuck Taylors--I go barefoot in our little gym here, but these travel with me
Deadlifting socks
Spare shorts and tee shirt and socks
2 tennis balls and 2 lacrosse balls
Resistance bands--these are new, but they'll go in the bag
Notebook when I travel
Chalk when I travel--I'll use it surreptitiously if I have to in places that forbid it. Heck, by the time they catch me, I'll be done, and I rarely go to the same gym more that 2 times anyway.
Small towel for drying myself or the sweaty bench, or cleaning up the evidence of chalk use

For those of you who are from outside N America, and who are chuckling about Shawn's "long athletic pants", you realize that we use the word "pants" to mean the same thing as "trousers", right?

LOL. I didn't know what else to call them. They aren't sweatpants. They are athletic trousers. hahaha

I also bring a log book and pen.

I like to be clean after I take a dump. It's the closest thing to taking a shower afterwards.
