I actually consider form deterioration to be failure. On something like deadlift or squats you get that feeling that you can't maintain form for another rep. So, I stop there and that to me is failure. Then there is something like bench press where not being able to do another rep happens at the sam...

Make sure you don't move your shoulders. If heavy weight makes you swing that might be the problem. Make sure your upper arms are still and you only bend at the elbows. Ryan, I disagree. I don't see how you can make gains without going to failure on at least the last set. If you go to failure on all, that just decreases how many sets you need. That is so true. The gym has been VERY crowded this week. It always seems to be like that too. I wish they would keep it up, but 90% of them don't.
