what is usually the cause of an elevated shoulder? is it due to tightness in the lats?

Your upper traps are tight and your seratus anterior and probably your rhomboids are weak. Do more push ups, chin ups and wall slides.

Froggy, don't use shoulder relax on other parts of your body. Won't work right.
